How To Apply To Access Super for Dental Work

DIY Applications

If you’re comfortable taking on the process yourself, you can download the forms from the ATO website. Fill them out with all the required documentation, including medical reports and treatment plans. Once everything is in order, submit your application directly to the ATO. If you choose to do this process, it usually takes longer for approval and you also require to see your medical GP for approval in addition to your dentist. In preparation for your application, you will need the following documentation:

  1. An Itemised Quote/ Treatment Plan: Get a detailed quote or unpaid invoice for the dental work needed to get your health back on track.
  2. Dental Report: Obtain a report from your dentist that explains why the treatment is necessary.
  3. Medical Report: Obtained by visiting your medical GP.
