Information on how to enter, collection of personal information and prizes form part of these conditions of entry. Entry into the promotion is deemed acceptance of these conditions of entry.

Australian residents, as defined by Australian Immigration requirements, who are 18 years of age or over are eligible to enter, subject to the other conditions. Employees of the Promoter and their immediate family members, including companies associated with this promotion and companies and contractors associated with the Promoter, including any prize producers are not eligible to enter.
Entrants must provide a valid email address and phone number to be eligible (so we can notify the winner of the result).
The promotion entry period commences at 12:00am on 1st July 2015 (AEST) and closes at 5:00pm on Friday 28th August 2015 (AEST) (“Promotion Period”).
How to enter:
Entrants become eligible for one entry into the competition by submitting the Customer Experience Survey.
The Promoter reserves the right to request winners to provide proof of age, identity, proof of residency at the nominated prize delivery address, and/or proof of entry validity. Proof of identification, residency and entry considered suitable for verification is at the discretion of the Promoter. The Promoter reserves the right to validate and check the authenticity of any prize claim or entry before awarding a prize.
It is a condition of entry to this competition that entrants consent to their name, email address, postcode and any further details required being retained and released by the Promoter for the purposes of advising you of the winners of this competition and for the purposes of emailing you notification of similar competitions and special offer promotions.
Entries will be deemed to be accepted at the time of receipt of the entrants submission and not at the time of transmission. Incomplete or incomprehensible entries will be deemed invalid. No responsibility will be taken for lost, late or misdirected entries.
Only one contest entry is allowed per eligible person. Entries may not be revised, edited resubmitted or replaced after the user clicks to submit an entry.
The winning entry:
This is a game of skill. Luck or chance play no part in determining the winning entry.
Entries will be judged one-by-one and the winning entry will be selected as the best combination of attributes following the following criteria: original, relevant and useful to most taxpayers, practical and possible for the Promoter to carry out.
The winning entry will be selected on or before Monday 31st August 2015.
The results of the competition and selection of the winning entry are final and no correspondence will be entered into. The winners will be notified by telephone and email within 2 working days of the winning entry selection. Should telephone contact be impossible, email will be used for notification. Winners will also be notified in writing that they have been selected as the competition winner.
Other entrants will be notified of the winner via the Lifestyle Smiles website and Social Media Sites.
The person who submits the entry deemed ‘The winning entry’ will receive an Apple Watch Sport Edition 38mm smartwatch valued at AUD $500 RRP.
The prize must be taken as offered and cannot be varied.
Unclaimed Prize Draw:
In the event that for any reason whatsoever a winner does not take an element of the prize at the time stipulated by the Promoter then that element of the prize will be forfeited by the winner.
A ‘second’ place winner will be chosen in the event that the original winner is not contactable or chooses not to claim or accept the prize after three months from the competition end date with the last day for entry being 28th August 2015. In this instance the original winner forfeits any and all claims to the prize, which will be awarded to the second best entry that meets all of the conditions stipulated above.
If required the second place winner will be notified by telephone or email within 2 working days. Should telephone contact be impossible, email will be used for notification. Winners will also be notified in writing that they have been selected as the second place winner.
The Promoter’s decision in relation to any aspects of the competition is final and binding on every person who enters and no correspondence will be entered into.
The Promoter and their associated agencies, and companies associated with this promotion will take no responsibility for prizes damaged or lost in transit. Once prizes have left Promoter’s premises, the Promoter will not be responsible for any delay in delivery, or failure of safe delivery of prizes.
The Promoter shall not be liable for any loss or damage whatsoever which is suffered (including but not limited to indirect or consequential loss) or for any personal injury suffered or sustained in connection with any prize/s except for any liability which cannot be excluded by law.
The Promoter is not responsible for any incorrect or inaccurate information or for any of the equipment or programming associated with or utilised in this promotion, or for any technical error, or any combination thereof that may occur in the course of the administration of this competition including any omission, interruption, deletion, defect, delay in operation or transmission, communications line or telephone, mobile or satellite network failure, theft or destruction or unauthorised access to or alteration of entries.
The Promoter reserves the right in its sole discretion to disqualify any individual who the Promoter has reason to believe has breached any of these conditions, or engaged in any unlawful, unfair or other improper misconduct calculated to jeopardise the fair and proper conduct of the promotion. The Promoter’s legal rights to recover damages or other compensation from such an offender are reserved.
By entering the entrant or referred entrant agrees to receive communications from Lifestyle Smiles Dental Clinics. All entries including the text, concepts or designs described in entries become the property of The Promoter. All entries will be entered into a database and The Promoter may use the entrant’s names, addresses and telephone numbers for future promotional, marketing and publicity purposes in any media worldwide without notice and without any fee being paid unless otherwise advised by the entrant. By entering the promotion, entrants confirm that they allow their details to be used for this purpose. If entrants do not consent to their details being used for future marketing purposes, the entrant should contact The Promoter on their details set out above.
If for any reason this promotion is not capable of running as planned, including due to infection by computer virus, bugs, tampering, unauthorised intervention, fraud, technical failures or any other causes beyond the control of the Promoter, which corrupt or affect the administration security, fairness or integrity or proper conduct of this promotion, the Promoter reserves the right in its sole discretion to disqualify any individual who tampers with the entry process, and cancel, terminate, modify or suspend the promotion. This is subject to any directions from a regulatory authority.
